- Behaviours of Concern Data: Summarised data on frequency, duration, and intensity of behaviours of concern and training provided to support staff. This information will include critical analysis of behaviours and the role of support workers within the participants realm
- Behaviour Support Plan: Implementation of comprehensive behavioral support plan. Regular reporting to Positive Behaviour Support Practitioner and Occupational Therapist. Reporting Restrictive practices where and if necessary to the NDIA Commision Portal
- Incident Reports and Risk assessments: Detailed and weekly/ reporting information regarding incidents, behaviours of concern and associated risk assessments. Ensuring transparency in communication via meetings with the participants stakeholders inclusive of Multi Disciplinary Team Meetings
- Sleeping and Overnight Support Data: Detailed sleep data that provides information on frequency and duration of the participants sleep regime observed overnight
- Sensory Reporting: Reporting and data collection of the participants sensory needs to relevant stakeholders
- Building of Daily Functional Capacity: Data collection of the everyday functional capacity that outlines the participant’s support needs over a 24hr period including morning, daytime, afternoon, evening and overnight
- Holistic Care Plan: This includes capacity building strategies implemented to improve the participants hygiene and everyday scheduling (developed collaboratively between the participant’s Psychologist, Speech Therapist, Positive Behaviour Support Practitioner and Occupational Therapist).
Genuine Disability Services is affiliated with NDIA and the State Government/ Department of Communities(Child Protection and Family Support). We are a team building service who will work collectively and collaboratively in a flexible approach to help support the participants in meeting their NDIS goals, Reasonable and Necessary taking into the consideration the participants dignity of choice and control.
